    • CPT Code 99453 (~$18 ) - Remote monitoring of physiologic parameter(s) – initial set-up and education
    • CPT Code 99454 (~$62) - Remote monitoring of physiologic parameter(s), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days
    • PT Code 99457 (~$51) & 99458 (~$42) or $136 for 1hr RPM - Effective Jan 2020, bill CPT 99457 for 1st 20 min of clinical staff time performing RPM activities & CPT 99458 for 2nd & 3rd 20-min increments.
